After making your purchase you will then be redirected to our secure govWorks platform where you will create a password and complete the DS-11 passport application form.
Here’s a list of the necessary documents you’ll need to apply for a new passport.
DS-11 form (completed online, printed single-sided, do NOT sign now)
One Passport Photo (sample picture)
Proof of U.S. Citizenship (list of examples)
Proof of Identity (list of examples)
Travel Itinerary, required for expediting service (list of examples)
Government Fees/Payments; Check payable to "US Department of State" for $170 if applying for a Passport Book Only and $200 if applying for a Passport Book and a Passport Card
Important: Do NOT sign the DS-11 application now. Please make 2 photocopies of all your supporting documents (proof of identity, passport photo etc.).
You will take your DS-11 application and all of the supporting documents to a Passport Acceptance Agent (AA), who are conveniently located throughout the country, normally at U.S. Post Offices or Courthouses.

The AA will check your ID, witness you sign the DS-11 form, and add their signature. The agent will then collect all of your supporting documents and seal them into an envelope with the signed DS-11 form.
Important: Inform the Acceptance Agent that you are using an expediting service to "Hand Carry" your passport application to the government for processing. Make sure the (AA) returns the sealed envelope to you so that you can FedEx it directly to APVI.
Write your full name, date of birth, and date of departure on the sealed envelope.
Place the sealed envelope in a FedEx envelope.
The second copies of your supporting documents and DS-11 form should be placed in the FedEx envelope alongside (but not in) the sealed envelope. This allows us to review everything for accuracy before delivering your application for processing. Submitting error free documentation will eliminate delays.
Ship the FedEx envelope to our office using the FedEx airbill provided by APVI.
We will process your new passport and FedEx it back to you.
